One of the biggest mistakes test takers make is reacting to the answers with their feelings instead of thoughtfully considering each answer using logic.

The following steps should be used when eliminating wrong answers:

1) Cover all other answers except the first one with a piece of paper.

2) Read the first answer. Then read your reworded question and decide whether or not it answers the reworded question. If it’s wrong, eliminate it. If you’re not sure, then leave it for later consideration.

3) Do step 2 for each of the answers remaining.

4) Consider what answers are left after you’ve made eliminations.

5) Read the question again to ensure that you’ve interpreted the reworded question correctly.

6) Decide which choice best answers the question.

Using the same question in the previous section, let’s eliminate the wrong answers and arrive at the right one:

A six year old girl has a fractured femur and her parents take her to the emergency room. Her parents replied, when asked how the injury happened, that she fell off the couch. During the exam ...
... the nurse finds several lesions on her buttock. Which statement would be most appropriate for the nurse to record from her findings?

a. Six lesions noted on buttocks at various stages of healing.

b. Multiple lesions on buttocks due to child abuse.

c. Lesions on buttocks due to unknown causes.

d. Several lesions on buttocks caused by cigarettes.

The reworded question might be, “What is good charting?”

1) Cover up all the answers except 1. Consider each answer carefully.

2) Read the first answer. Decide if this choice, “Six lesions noted on buttocks at various stages of healing” answers the question, “What is good charting for this situation?” Perhaps it does. Leave it for later consideration.

3) Repeat step 2 with each choice:

b. “Multiple lesions on buttocks due to child abuse.” This is not good charting because the nurse is judging whether or not it’s child abuse. You can eliminate this answer.

c. “Lesions on buttocks due to unknown causes.” This might be good charting, so leave it for later consideration.

d. “Several lesions on buttocks caused by cigarettes.” This is not good charting because the question doesn’t say how the burns occurred.

4) The two choices remaining are a and c.

5) Read the question again. It asks you to identify good charting.

6) Which choice is the best answer: “Six lesions on buttocks at various stages of healing,” or “Lesions due to unknown causes”? Knowing that good charting consists of information that is correct, precise, objective and thorough, will help you make the right choice, answer a. Don’t be fooled into choosing c, by questioning your own ability to be sure of the different stages of healing.

The question asks you to choose the answer that proves your knowledge of good charting. You must choose the answer that demonstrates your ability to practice safe and effective nursing. Always focus on the question.
