What Medicare Claims Data Shows About Talzenna Utilization
Talzenna (talazoparib) is an oral PARP inhibitor used in specific patients with advanced breast or prostate cancer. As its use has expanded, Medicare claims data can provide a useful view of how the drug is being utilized among beneficiaries with prescription drug coverage. Claims data can help quantify prescriptions, beneficiaries, spending and prescribing patterns, although it cannot by itself explain why an individual patient received treatment or whether the treatment was clinically successful.
Talzenna’s role in cancer treatment
The FDA first approved Talzenna in 2018 for adults with deleterious or suspected deleterious germline BRCA mutated, HER2 negative, locally advanced or metastatic breast cancer. The approval was supported by the EMBRACA trial, where Talzenna produced a median progression free survival of 8.6 months compared with 5.6 months with the physician's choice of chemotherapy.
In 2023, the FDA expanded Talzenna’s role in prostate cancer.
Talazoparib, in combination with enzalutamide, was approved for adults with homologous recombination repair (HRR) gene mutated metastatic ...
... castration resistant prostate cancer. In the HRR mutated population studied in TALAPRO 2, the combination significantly improved radiographic progression free survival compared with enzalutamide alone.
These indications are important when interpreting Medicare utilization because Talzenna is a biomarker directed therapy rather than a broadly used cancer medication.
What Medicare claims data can tell us
CMS Part D data captures prescription drugs dispensed to Medicare beneficiaries enrolled in Part D. The CMS Prescription Drug Profiles Public Use Files represent 100% of Part D prescription claims, subject to privacy related suppression, and include information such as beneficiary characteristics, drug information, prescriber characteristics and payment measures. (Centers for Medicare & Medicaid Services)
CMS's Part D Prescriber datasets provide another level of detail. They can show the number of prescriptions dispensed, including refills, standardized 30 day fills, days supplied and total drug costs associated with a prescriber and drug. However, CMS suppresses certain low volume records to protect beneficiary privacy. (CMS Data)
For Talzenna, this means claims analysis can help answer questions such as how many prescriptions are being dispensed, how utilization varies geographically, which types of providers are prescribing the drug and how Medicare spending changes over time.
Utilization reflects a specialized patient population
Talzenna utilization should be interpreted in the context of its relatively narrow treatment population. For breast cancer, eligibility is tied to germline BRCA mutation status and HER2 negative disease. For prostate cancer, the FDA approved combination therapy for HRR gene mutated metastatic castration resistant prostate cancer. (U.S. Food and Drug Administration)
Consequently, prescription volumes should not be interpreted in the same way as utilization for medications prescribed across a much broader population. A lower number of claims can reflect the drug's biomarker defined indications rather than limited clinical relevance.
Medicare utilization data can also reveal the financial scale associated with this type of therapy. CMS's Medicare Part D Spending by Drug dataset reports spending for drugs prescribed to Part D beneficiaries and includes information on average spending per dosage unit and changes in spending over time. The latest annual CMS dataset currently available is for 2024. (CMS Data)
More recent quarterly data can provide an additional directional view. Public summaries based on CMS data report approximately $124.5 million in 2025 Part D spending for Talzenna, with about 1.8 thousand claims and 762 beneficiaries in the first quarter of 2026, when the data are aggregated across Talzenna products. These figures should be treated as utilization and spending indicators rather than measures of treatment effectiveness.
Claims data can reveal access and prescribing patterns
Medicare claims analysis becomes particularly valuable when utilization is examined alongside coverage and prescribing requirements. For example, Medicare plans may use prior authorization or other utilization management approaches for specialized oncology drugs. Public Medicare plan information shows Talzenna is subject to utilization management in some formularies, illustrating how coverage rules can influence access.
Claims data can also help identify geographic differences in utilization and prescribing concentration. Researchers can compare prescription counts, standardized fills, beneficiary numbers and spending across states, counties or provider groups. Such analysis can help pharmaceutical organizations understand where treatment adoption is concentrated and where additional investigation may be warranted.
What claims data cannot show
Medicare claims data has important limitations. A prescription claim does not establish that a patient took the medication as prescribed, responded to treatment or experienced a particular outcome. It also does not provide the complete clinical context behind prescribing decisions.
CMS notes that Part D prescriber data only represents beneficiaries enrolled in Medicare Part D and should not be considered representative of a physician's entire practice. The data are also not intended to measure the quality of care. (CMS Data)
Therefore, claims data is most useful when combined with other sources, such as clinical information, laboratory data, diagnosis information, treatment history and outcomes data.
The value of Medicare data for Talzenna analysis
The growing availability of Medicare claims data creates an opportunity to examine Talzenna utilization at a population level. Analysts can track prescription activity, beneficiary counts, spending and provider patterns while placing those findings within the context of FDA approved indications and evolving oncology treatment.
For pharmaceutical teams, the real value comes from connecting utilization data with the clinical questions behind it. A Medicare claims analysis can help identify where Talzenna is being used, how utilization changes over time and what the broader treatment landscape looks like. When claims are appropriately linked with other healthcare data, they can provide a more complete view of patient journeys and treatment patterns across the cancer care continuum.
CareSet helps pharmaceutical organizations work with Medicare claims data to develop actionable insights into treatment utilization, patient populations and healthcare trends. By analyzing Medicare data in the right clinical context, organizations can move beyond prescription counts toward a deeper understanding of how therapies such as Talzenna are being used in real world care.
