Medical coding basically refers to the process of assigning formal, standardized medical codes to the patient medical records. When a patient visits a clinic or hospital, a medical record is created. This medical record will include patient identification information along with a description of the chief complaint, a diagnosis, any procedures that were performed, medications prescribed, and follow-up instructions. The diagnostic and procedural information contained in the patient medical record are converted into simplified numerical codes by trained medical coders. These codes allow quick recognition of complex medical procedures and diagnoses by the healthcare computer systems, which are then electronically processed for payment by third party payers or Insurance companies.
Medical coding involves review of patient medical records for the identification of reimbursable or billable events and activities. Once a billable event has been identified, the accurate numerical code is put into a healthcare reimbursement system.
