Why Bacterium is prescribed (Indications):

This medication is a combination of trimethoprim and sulfamethoxazole, prescribed for certain types bacterial infections. It is also used to treat traveler’s diarrhea. It eliminates bacteria that cause various infections.

DRUG CLASS AND MECHANISM:

Bacterium is a combination of two synthetic (man-made) antibiotics, sulfamethoxazole and trimethoprim. Both drugs reduce the ability of some bacteria to utilize folic acid for growing. Sulfamethoxazole is an anti-bacterial sulfonamide, a "sulfa" drug. It disrupts the production of dihydrofolic acid while trimethoprim disrupts the production of tetrahydrofolic acid. Dihydrofolic acid and tetrahydrofolic acid are forms of folic acid that bacteria and human cells use for producing proteins. Trimethoprim inhibits production of tetrahydrofolic acid by inhibiting the enzyme responsible for making tetrahydrofolic acid from dihydrofolic acid. By combining both drugs, two important steps required in the production of bacterial proteins are interrupted, and the combination is more effective than either drug alone.

Contraindicated in patients with anemia, hypersensitivity, during pregnancy and breast-feeding.

Category C: Animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks.

DRUG INTERACTIONS:

Sulfamethoxazole/trimethoprim can enhance the blood-thinning effects of warfarin (Coumadin), possibly leading to bleeding. Sulfonamides such as sulfamethoxazole can increase the metabolism (break-down and elimination) of cyclosporine (causing loss of effectiveness of cyclosporine), and can add to the kidney damage caused by cyclosporine.

All sulfonamides can crystallize in urine when the urine is acidic. Since methenamine (Hiprex, Urex, Mandel amine) causes acidic urine, it should not be used with sulfonamides.

Dosage & When it is to be taken:

It is a fixed combination product containing sulfamethoxazole and trimethoprim in 5:1 ratio.

Adult- PO- The recommended dose is 2 tablets every 12 hours for a period of 10 to 14 days.

Child- The recommended dose is 8 mg/kg of trimethoprim and 40 mg/kg of sulfamethoxazole daily in 2 divided doses every 12 hours.

How it should be taken:

It comes as a tablet and liquid to take by mouth, with or without food.

Warnings and Precautions :

Caution should be exercised in patients with history of liver or kidney disease, asthma, severe allergies, glucose-6-phosphate dehydrogenase (G-6-PD) deficiency (an inherited blood disease), any allergy, who are taking other medications.
Avoid exposure to sunlight.

Side Effects:

Most Common - Nausea, vomiting and loss of appetite.
Blood - Decrease in blood cell counts.
Allergic - Skin inflammation, eruptions, rash, itching, redness, hives, bruising and photo sensitivity.
Gastrointestinal - Liver inflammation / damage, jaundice, hepatic necrosis, elevation in liver enzymes, inflammation of intestine/ pancreas/tongue, mouth ulcer, abdominal pain and diarrhea.
Genitourinary - Kidney failure, Blood Urea Nitrogen and absence of urination.
Central Nervous System - Brain inflammation, convulsions, in coordination, fainting, ringing in the ear, headache, hallucinations, depression and anxiety.
Metabolic - Low blood sugar.
Musculoskeletal - Joint/muscle pain.
Respiratory - Cough shortness of breath and lung infection.
Miscellaneous - Weakness, low energy and insomnia.

Other Precautions:

Avoid excess dosage.

Storage:

Store at room temperature. Protect from heat, light and moisture. Keep away from reach of children.
