As far as health care is concerned, you can work in hospitals or private clinics and assist doctors and nurses or assist patients in having their food and taking their medicines at the right time, making as well as changing their beds, and so on. On the other hand, social care may include helping people who are physically challenged, assisting people with learning disabilities or helping those who are suffering from age related problems. This may include helping them to dress up in a proper manner, taking them out for an evening walk if they cannot go out by themselves, taking them out for shopping, and so on.
