This decision is actually ...
... up to the dentist, after careful visual examination and with the use of technology such as digital and panoramic x-rays a dentist will arrive to the position of whether or not to extract a structure, if the patient is in pain it is obvious that the tooth has to be distracted because a position it is in can be affecting nerves and the rest of the teeth.
Since the extraction of wisdom teeth is actually very painful most dentist will resort to heavy sedation or they may opt to apply general anesthesia to a patient, this way while the patient sleeps the third molars can be removed in a virtually painless way.
When it comes to wisdom teeth or to third molars is of the utmost importance to never wait until it is absolutely necessary to go to the dentist because the longer a person waits to have the wisdom teeth removed the bigger the chances are of complications this is multitude of fact that as a person ages the teeth fuse with the jawbone and as previously said their extraction may result in unexpected complications.
