Letter To Credit Agency
Experiencing conflict with credit agencies is no walk in the park. What greater nuisance than to discover that all of your efforts to be in good standing with credit agencies have gone in vain? You must keep recent accounts of the activity on your credit report. This credit report contains vital information such as where you live, whether you've been sued or arrested, if you've filed for bankruptcy and how you pay your bills. This item (identify item(s) disputed by name of source, such as creditors or tax court, and identify type of item, such as credit account, judgment, etc.) is (inaccurate or incomplete) because (describe what is inaccurate or incomplete and why). I am requesting that the item be change (or that it be removed) to correct the information.
Enclosed are copies of (here is where you add all evidence such as payment records and court documents) supporting my position. Please re investigate this (these) matter(s) and (delete or correct) the disputed item(s) as soon as possible.

You may want to mention in the letter that you are familiar with the Fair Debt Collection Practices Act, Section 809: If the consumer notifies the debt collector in writing within the thirty-day period described in subsection (a) that the debt, or any portion thereof, is disputed, or that the consumer requests the name and address of the original creditor, the debt collector shall cease collection of the debt, or any disputed portion thereof, until the debt collector obtains verification of the debt or any copy of a judgment, or the name and address of the original creditor, and a copy of such verification or judgment, or name and address of the original creditor, is mailed to the consumer by the debt collector.
Revealing that you are knowledgeable lets credit agencies know that you are serious about the matter and this promotes them to handle you accordingly.
