What Is Payment Posting in Medical Billing?
Payment posting is the process of recording payments and related financial information from insurance companies and patients into a practice management or billing system.
For insurance payments, billing teams typically work with an Explanation of Benefits (EOB) or Electronic Remittance Advice (ERA). These documents provide details on the amount paid, adjustments, denials, patient responsibility, and other claim-level information.
The procedure for payment posting in medical billing links the payer's claim decision to the patient's account and the provider's financial books. Accurate posting clarifies balance status and helps identify claims that need further attention.
Payment Posting Process in Medical Billing
A consistent payment posting process in medical billing generally involves the following steps:
Receive payment and remittance information
Collect EFT, ERA, EOB, check, or patient payment information.
Match the payment to the correct account and claim
Verify patient, payer, claim, date of service, and other relevant information.
...
... Post the payment
Enter the payment at the appropriate claim or service-line level.
Record adjustments and patient responsibility
Apply contractual adjustments, deductibles, copayments, coinsurance, and other applicable amounts correctly.
Identify denials and exceptions.
Route denied, underpaid, or unusual claims for appropriate follow-up.
Reconcile the payment
Compare posted amounts with the corresponding deposits and remittance information.
CMS explains that EFT and ERA transactions may arrive separately and need to be reassociated so the payment can be reconciled and posted correctly to accounts receivable.
Common Payment Posting Errors
1. Posting Payments to the Wrong Account
One of the most basic but potentially disruptive errors is applying a payment to the wrong patient, claim, payer, or date of service.
This can make one account appear paid while leaving the correct account with an incorrect balance. Verifying multiple identifiers before posting can help prevent this problem.
2. Entering the Wrong Payment Amount
Manual data entry can result in transposed numbers or incorrect amounts. For example, entering $1,350 instead of $1,530 can create reconciliation differences and distort the patient's account.
Electronic posting can reduce manual keying, but exceptions and manually processed payments still require careful review.
3. Misclassifying Adjustments
Not every reduction in a claim balance should be treated the same way. Contractual adjustments, patient responsibility, denial-related balances, and other adjustments need to be classified appropriately.
Incorrect adjustment posting can overstate patient balances or hide amounts that need further follow-up.
4. Failing to Identify Denials
A zero-payment claim is not necessarily a claim that requires no action. If the remittance contains a denial or remark code and the posting team closes the balance, it may miss an opportunity to correct or appeal the claim.
Effective posting should make denials visible to the appropriate follow-up team.
5. Leaving Payments Unapplied
Payments that cannot be immediately traced to the proper account will be put in a suspense or unapplied account.
Although this may be the proper procedure for some time, it becomes a problem because it makes the accounts receivable unreliable.
6. Missing Underpayments
Payment posting provides another chance to detect any underpayments made by the payer. This is only possible if the teams don't just enter payments into the system but also review the remittance advice and contractual obligations.
The latest RCM guidelines place increasing emphasis on payment posting as a crucial control point.
7. Delayed Payment Posting
When payments remain unposted for several days, account balances may not reflect the latest payer activity. This can affect AR reporting, patient statements, follow-up work, and financial visibility.
How to Prevent Payment Posting Errors
Preventing errors requires a combination of standardized processes, technology, quality checks, and trained staff.
Use ERA-Based Posting Where Available
Electronic remittance information can reduce repetitive manual data entry and support faster posting. However, automation should still include appropriate validation and exception handling. Recent industry guidance emphasizes that automated posting works best when payer mappings, adjustment rules, and exception workflows are properly controlled.
Verify Before Posting
Staff should confirm the patient account, claim, payment amount, date of service, payer, and relevant adjustment information before finalizing a posting.
Reconcile Payments Regularly
Compare payment records with bank deposits and remittance information. CMS identifies reassociating EFT and ERA information as an important step in accurately reconciling healthcare payments.
Create an Exception Workflow
Route unapplied payments, unusual adjustments, denials, credit balances, and significant payment variances into defined work queues rather than posting them and forgetting them.
Monitor Posting Accuracy
Healthcare organizations can monitor metrics such as posting turnaround time, unapplied cash, reconciliation variances, correction rates, and unresolved exceptions to identify process weaknesses.
When Should Medical Practices Consider Outsourcing Payment Posting?
Payment posting can become difficult for practices managing large claim volumes, multiple payers, paper remittances, electronic payments, and frequent exceptions.
Outsourcing can provide access to dedicated payment posting staff and structured workflows while allowing the internal team to focus on other revenue cycle activities. A specialized partner can also support reconciliation, exception identification, denial-related follow-up, and payment accuracy as part of a broader RCM workflow.
e-care India can help health care organizations with payment posting and other revenue cycle processes, keeping accounts up to date.
