Keep The Source Beside The Health Headline

A health headline is easy to forward. Finding the paper behind it can take longer than reading the news story itself. That missing step matters: a reader should be able to tell which document the headline is talking about.
Start with the source link. Does it open a research paper, a university announcement, a conference page, or another news story? Write down what you actually found. A press release can help locate a paper, but it is still a separate document with its own wording.
The date deserves a second look too. A story published this morning might discuss a paper from months ago. Keep both dates in your notes. If the paper has an update or correction, read that before saving a quotation. Small detail, easy to miss.
Next, put the headline beside the paper's title and abstract. Look for the people, setting and outcome mentioned in each. If the news story leaves one of those out, keep the missing detail in your own summary. There is no need to turn a short note into a textbook; a few extra words can make it much clearer what was actually studied.

... copy what it measures and the time period as well. Without those, the number is hard to check later. Dont fill a gap with a guess just because the headline sounds certain.
A useful reading note can be very plain: paper title, publication date, source location, and one sentence describing the question. Add a seperate line for what you could not verify. If only an abstract is available, say so. A blank space in the evidence trail is more useful than a confident sentence that nobody can trace.
