Grades of baldness: Norwood classification

Class I: Entire hairline lies high on the forehead and is not actually balding.


Class II: Triangular areas of recession in the sides of frontal area and minimal recession on mid frontal area.


Class III: Borderline case. Deepening of triangular recession or can include thinning of hair on the vertex.


Class IV: Further frontal hair loss and widening of hair loss from vertex but still a wide band of hair present separating frontal and vertex hair loss.


Class V: Widening of frontal and vertex hair loss and continual breaking of separation line of hairs between frontal and vertex baldness.


Class VI: Band or bridge of hair disappear merging frontal and vertex baldness.


Class VII: A band of hair presents only in back and side of the head.


What are the methods of hair transplant?


The hair transplant is performed using 2 methods each one differ in their way of harvesting the hair grafts and are explained as follows:


1. Strip technique or FUT hair transplant: FUT hair transplant is performed by excising a thin strip of skin from the donor area that is divided into sections for harvesting individual hair graft. Each hair graft is transplanted at the desired bald area carefully without damaging the hair root and while maintaining the hairline. The donor area is closed and sutured for faster healing with almost invisible scarring.


2. FUE hair transplant: FUE hair transplant is performed by extracting each hair graft from the donor area using a punch like device. The punch is inserted into the skin of the donor area at a certain depth and angulations in order to extract the hair graft with no damage. The hair graft is pulled from the skin and then transplanted at the desired bald area carefully with minimizing the damage to the hair grafts.


Is FUT hair transplant the best option for high grade baldness? Why?


Each hair transplant technique has certain indications based on which the technique is chosen and implemented as per the situation of the patient. The choosing of the right technique depends on the age of the patient, sex of the patient, need of the hair density, availability of the hair density at the donor area, area of baldness and quality of the hair grafts. Out of all factors the need of the hair density is a very important factor deciding the choice of the technique.


If the need of numbers of hair grafts is high then FUT is the best suitable technique indicated and on other hand when the need of numbers of hair grafts is low then FUE could be a suitable technique. FUT hair transplant technique could successfully harvest around 3000 – 3500 hair grafts whereas FUE technique could successfully harvest 2000 – 2500 hair grafts.


Therefore, FUT could be suitable technique in extensive or high grade baldness cases and where there is demand of high density hair transplant. For high grade baldness like grade VI or VII, Combination technique of FUT and FUE is a suitable technique as the technique could successfully harvest around more than 4000 hair grafts in single session. In Combination technique, both FUT and FUE are performed in single session out of which the major count of hair grafts are harvested using FUT technique and the residual number of hair grafts are extracted using the FUE hair transplant.


It is very important to choose the right technique for the hair transplant based on the need of the patient so as to avoid the over harvesting of the donor site which would violate the safe zones. If the safe zones are violated it will definitely have negative impact on the permanence of the outcomes.
