Thyroid disorders refer to a rather dysregulated functioning of the thyroid gland, resulting in either an underproduction (hypothyroidism) or an overproduction (hyperthyroidism) of the thyroid hormones, triiodothyronine (T3), and thyroxine (T4). If thyroid disorders are left undiagnosed or inadequately treated, they can make it difficult for you to get pregnant, and later on may cause problems during your pregnancy.
Hypothyroidism and Pregnancy
Hypothyroidism is very commonly encountered problem in women these days, many a times found as an incidental finding on preliminary investigations. The common symptoms include; frequent heavy menses, lethargy, weight ...
... gain & cold intolerance. It has also been linked to irregular ovulation, increased risk of miscarriages, stillbirths & premature births. If you have an undiagnosed and/or untreated under active thyroid ie hypothyroidism, you may find it hard to conceive.
Once you are planning for a pregnancy you should let your doctor(s) know, who will most likely recommend that thyroid levels be ideally be maintained in the lower half of the reference range even before you get pregnant. During pregnancy, you may need higher doses of medication (thyroxine), to maintain normal levels of the thyroid hormones; starting as early as the confirmation of pregnancy itself. Thereafter regular blood tests should be done throughout the entire course of pregnancy for any dose titrations, if necessary.
Hyperthyroidism and Pregnancy
As far as an over-active thyroid or hyperthyroidism is concerned, Grave’s disease is the most common cause. It is an autoimmune disease that affects women more than men. But if left untreated, it has a negative impact on fertility in both the female and the male partner.
Affected females may complain of lighter irregular periods, irritability, increased sweating, weight loss despite good appetite & heat intolerance. It may disrupt the normal menstrual cycles and hence interfere with conception. During pregnancy, it has found to be associated with increased risk of abortions & preterm births.
In men, hyperthyroidism has been known to cause a marked reduction in sperm count, resulting in diminishing fertility. However, sperm count usually get back to normal once the condition has been treated.
In case you’re trying on have a baby, do so after taking the due treatment for this disorder, and follow up with blood tests to see your thyroid function. Patients are typically kept on lowest effective dose of medications to maintain the hormone levels in the normal range. Even after pregnancy is established, you could still have Graves’ antibodies in your blood and these can potentially affect your baby, hence it is important to let your obstetrician know about it beforehand.
Getting you thyroid checked, especially when seeking treatment for infertility & finding the right treatment plan will not only increase your chances of pregnancy but also carrying it to term successfully.
Apart from a timely diagnosis & treatment, there a few lifestyle changes that may help in maintaining a normal functioning thyroid. These include reducing stress levels, eating a well-balanced diet, avoiding smoking & alcohol and incorporating exercise into your daily routine.
