Here are two key rules when coding for lesion excision:


Include margins in measurements. If you code based on lesion size, you are cutting yourself off at the knees. You should measure each excision at the widest diameter, including any margins.


Measure lesion size pre-excision: See to it that you make the measurement before the FP removes the lesion. You should not report (lesion and margin) size from the pathology report. The sample that you send to pathology will be smaller than the one you get back.
