History of healthy
The birth of the child was normal since he did not have any illness. The child is not undergoing any medication at the moment, and so she is in good conditions of healthy. The vision, hearing as well as the motor functioning are within the normal range. The healthy observations took place twice and thus they are adequate and valid for the scoring judgments.
Education History
The child is currently in school and is taking Bachelor of Science, his teacher is Dr. Azar. The child is proficient in English speaking and has ease of understanding instructions. The child can also use Arabic. He is very fast in problem-solving and provides good responses to questions.
Developmental assessment
Adaptive
Has smooth suckling with coordinated movements and makes use of two hands in holding the breasts or a bottle. He can take semisolid food as well as strained food from a spoon and ...
... swallow it. The child can use gestures to ask for food, and he can also dress himself. The child can feed himself and also put on shoes without any assistance. The child needs assistance when blowing her nose, but in drying his hands, he needs no assistance. He is unable to choose the appropriate utensils for food and may not respond to instructions given. Someone has to remind him to take appropriate actions. He can choose the appropriate clothes for the weather.
Personal-Social
He demonstrates other people’s awareness through vocalization and smiling at the age of eleven months. Between the ages of twelve to seventeen months, the child was aware of his feet and could show a desire for social attention. Between eighteen and twenty-three months, he could discriminate between people that were familiar and those not familiar. At age four he could not allow others to take part in his activities, and could not engage well in adult role playing. At age five the child would not follow adult directions without assistance and could neither state his first or last names. He could not recognize adult feelings or cooperate in group activities at age seven. He also had a problem in discriminating between socially acceptable and unacceptable behaviors. He could wait for his turn for the attention f an adult.
Communication
The child could have an attraction to a familiar voice and produce differentiated cries at age zero to eleven. At age twelve to seventeen, he could respond to various tones of voices and produce some consonant and vowels of a single syllable. At eighteen to twenty-three months of age, he could identify familiar members and vary his voice. At age three and five years, he could respond to who and what questions, where and when, repeat familiar words with clear articulation and initiate actions related to the environment. At age six to seven years, he had a problem with conversing on topics that entailed over five turn-taking exchanges, but he would identify a word from the definition and follow three step verbal commands.
Motor
Up to eleven months of age, he could maintain an upright posture without assistance; hold hands in loose-fisted manner when he is not grasping an object. He could retrieve a small object using hands and transfer an object between hands at the age of between twelve and seventeen. He could stand at age eighteen to twenty-three months and intentionally propel an object. From two years and four years of age, he could maintain his balance when moving, run without falling, scribble patterns spontaneously, but could not walk forward 2 or more steps on a line alternating feet.
Recommendation
Saeed had a normal birth, and he did not have any illness or allergies during birth. He can learn well and respond well to issues in the required way. His growth and interactivity are normal and lively. He only needs to learn to participate in conversations entailing more than five turn-taking exchanges. He does not need any referrals.
Conclusion
As per the recommendation, I hope Saeed will participate fully in turn-taking conversations entailing over five exchanges. He should also be able to recognize adults’ feelings and respond appropriately. She does not need any medication because she is ok, and the assessment took place in the right manner.
