Personalized care and support for the elderly is a systematic process which is entirely based on what the patient needs and how the care shall be molded in order to meet the healthcare, social, and physical and psychological needs of the patients.

Better conversations between the healthcare practitioner and the patient or the patient’s family members can help create better personalized care and support plans.

The overall aim would be to assess, analyze what is important for the patient and how the patient's health can be improved or monitored.

What is important for the patient to achieve a good life that needs to be taken into consideration while creating a healthcare and emergency plan.

What programs or activities shall be introduced to keep the patient busy or entertained?
What care will be provided?
Details of the care and support plan (daily, weekly, in emergency cases)

Only after needs assessment, a perfect plan will be created.

The plan which is created shall be reviewed or revised continuously as per the emerging needs or the patients. Some times, carers do not focus on changing behavior or the patient and continue to deal with the patient following the same routine. This approach wil lead to a lot of disturbances and the patient may suffer.

Therefore, it is necessary to make changes in the plan or create a new plan from time-to-time as per the changing behavior for the elderly patient. Of course, health care needs, medical needs may not be altered often but other physical activities, programs or other important things a patient shall be kept busy with shall be changed from time-to-time so that the patients can have a great time.
