While much good can be said of growing older, the prospect of physically aging leaves much to be desired. One of the harshest realities of the process of physical aging is the damage that time and life can cause to the face. As people age, their facial skin grows less elastic, and visible flaws become more and more prominent as the years pass. For those seeking to maintain their physical beauty into old age, rhytidectomy – a surgical operation better known as the facelift – becomes an ever more attractive option. A facelift is a cosmetic surgery operation intended to improve the firmness and smooth texture of the facial skin, thus reducing the appearance of the outward signs of aging. Although some surgeons require that the patient be sedated and under general anesthesia when a rhytidectomy is being performed on them, the typical facelift operation is performed under local anesthesia, which is the preferred option. During the procedure, the patient remains awake and aware in a relaxed and pain-free state. The procedure itself requires that the surgeon detach the skin of the patient’s face and neck, allowing the underlying tissue and muscle to be tightened; some liposuction of excess fat may also be required. The skin is then placed back upon the face and neck and stretched taut. Excess skin (often including that found upon the patient’s eyelids) is then cut away and removed, the skin is sutured into place, and bandages are applied to the wounds. Although the procedure is exacting and complex, it has a very long history and can be performed by a competent surgeon in only a few hours, often on an outpatient basis. Complications of a rhytidectomy can be severe. These may include Hematoma Infection Adverse reactions to anesthesia Damage to the facial nerves Necrosis of the skin Major bleeding However, dangerous complications are rare. The use of tobacco and certain medications (e.g. aspirin) are forbidden during the recovery period due to their promotion of increased bleeding. A facelift operation, while clinically routine, is nevertheless a major surgical procedure and carries with it certain serious risks, including the risk of premature death. Aging is not kind to our faces. The combined effects of wind, weather, illness, and the stresses of life act to compromise its ability to retain its very shape. The cells themselves within our skin and flesh eventually fail, losing their stretch and give, imprinting upon our faces the tracks of time that we call wrinkles. These folds and flaws are acceptable to some; for others, however, they are a disfigurement, a distortion of their true face – something to be corrected. And a way exists to correct them: a surgical technique called rhytidectomy – more commonly known as a facelift. This cosmetic surgical technique can firm and smooth the face by altering the skin and supporting tissue, improving its appearance. A facelift operation is usually performed under a local anesthetic on an outpatient basis, although hospital admission and a general anesthetic are used in some infrequent cases. During a typical procedure, the patient remains relaxed, awake, and aware, but feels no pain during the entire operation, which can last up to several hours. The procedure itself is literally a lifting of the face. The surgeon cuts the facial skin free and lifts it away, revealing the tissue beneath. This is trimmed and tightened, and excess fat is removed if necessary along with extra skin. The skin of the eyelids and other associated areas may also be trimmed and tightened during the procedure. Once the underlying tissues are treated, the skin is tightened over the face and sutured into place. As might be expected with such a complex surgical procedure, complications are common with facelifts, though these are usually manageable. Reaction to anesthesia is among the most serious of these, although this is a rare occurrence; damage to facial nerves and skin necrosis are similarly rare. More frequent complications include hematoma and secondary infections. Tobacco, aspirin and certain medications can increase the risk of bleeding, and patients may not use these during the recovery period. Despite the undeniable risks involved, the rewards of facelift surgery can be more than worth the risk. The new lease on life that a person with severe facial damage can experience as the result of a facelift is often near-miraculous. In addition, the improvement in self-esteem that can come from getting a new look can create a complete psychological turnaround in a patient who has suffered from the negative effects of facial scarring due to age. Of course, any decision regarding facelift surgery must not be made lightly. As it happens, such a form of cosmetic surgery does exist. It is called facial implant surgery. In facial implant surgery, the surgeon rejuvenates, augments and reconstructs the face through the use of implants. For the patient seeking to improve or repair the appearance of the face through facial implant surgery, three primary surgical procedures are available: Chin Implant Procedure: Used to enhance the jaw line, this is an augmentative procedure that is often combined with other facial operations as part of achieving an overall look. Custom implants may be placed during outpatient procedure via incisions made beneath the patient’s chin or the inside of the mouth at the junction of the gum and lip. Jaw Implant Procedure / Mandibular Augmentation: Intended to strengthen the patient’s jawline, this operation requires a thorough examination and mapping of the jawbone prior to beginning the procedure. The best location for the implant (usually atop the mandibile) is then determined, the initial incision is made, and the implant is affixed with sutures, titanium screws, or some combination of these. The surgeon then examines and adjusts the position of the implant as necessary to create the look best suited to the patient’s face. The complete procedure usually takes from ninety minutes to two hours to complete. Cheek Implant Procedure: In this operation, the appearance of the cheekbones is strengthened by affixing implants on or below the cheekbones themselves. This is accomplished through two tiny incisions made just below the lower eyelids or above the top lip. Pockets are surgically created in the patient’s flesh, after which the implants are placed and secured using dissolving sutures and/or titanium screws. Complications from facial implant procedures are rare, and side effects are uncommon. Possible side effects include A tight, “drum-like” feeling in the area adjacent to the implant Swelling, usually lasting 1-3 days Stiffness of mouth and difficulty in chewing Inability to eat solid food (if surgery involves incisions within the mouth) Changes in facial structure due to facial implants will begin immediately after surgery, with the patient reaching his/her desired appearance in 12-16 weeks.
