Summary of information gathered
The person I interviewed is a close relative whose present health is good though she cited having an early warning of high blood pressure three weeks before the date of individual health assessment. She stated that her blood pressure was measured as 150/96 which is above the optimum level of 120/80 and 140/90. Her past health history revealed that she had not had any serious issues. The cause of the rise in blood pressure could be occasional cases of increased body weight, alcohol consumption, and stress which are evident from the history. She has not taken any medication for the condition in the past since the doctor advised her to adopt a healthy lifestyle and weight control. Her family history revealed that her father was diagnosed with high blood pressure, and her elder sister, who is a chronic alcoholic, was diagnosed with atherosclerosis.

The person I interviewed was not willing to share the information at first until I explained all the details to her and also assured her of confidentiality and use of the data for this assessment only. I had to discuss the purpose of the assessment and the need to share accurate information with me. Some sections of the interview were challenging especially in gathering the present health history as well as the family history. I observed that she did not want to share all the details about her health history since it was associated with that of her father and elder sister. For the section on present health history, I took longer time than expected since she took me round in circles without providing direct responses as to her blood pressure levels and its likely cause. It was also challenging to explain the cause of stress in her life since it was associated with the loss of her second child that was almost luring her to depression.

I was not as comfortable as I thought the experience would be since my patient was a close relative and her health condition affected me. I think that taking a health history a relative requires adequate preparation regarding psychological and emotional aspects both to the interviewee and interviewer. I had to apply certain interviewing techniques to ensure that I gather as much relevant information as possible. Interviewing can be challenging due to the discomfort of asking unexpected questions, hence necessary to learn the appropriate skills. The interviewing technique skills that I used include gathering as much information as I could, managing emotions and stress, asking relevant questions, and being adequately prepared. It was important that I gather as much information as I could though it was challenging. The acquired information would be essential in assessing the patient’s health history as well as any underlying factors that contributed to the present health condition.

The technique of asking as many questions as much as I could, help in relieving stress and emotions for the interviewee, and also created a rapport for the assessment. I had to ask only the relevant questions to enhance confidence in the entire process and also ensure that the patient was comfortable sharing the specific details of her health. It was not easy to acquire specific and relevant information from the patient due to the fear that her health condition would be exposed to other people. However, I assured her that I would not use any identifying information to the data collected and that it would not be availed to other parties unless for the purpose of the assignment. I also had to have adequate preparation before the interview to ensure that I research on best tips during an interview process and also how best to approach patients whenever they present to a care facility with a clinical issue. I also ensured that I consulted a person whom I was conversant with and would be easier to share her health details with other than a total stranger. Before the interview process, we had a short session of general talk to ensure that she was ready and composed for the process.
